You can do this by setting up a custom form template. Check the documentation for the steps.
I thought I would see the username as the first field but it says your URL?

You need to enable the fields that you want to display on the form. You can do that in the plugin settings.
I did do that :). I wouldn’t be submitting a ticket if it worked when I enabled the field. The field is enabled and is not showing up
When you’ve been doing this as long as I have, you don’t assume anything 🙂 Try disabling the setting “Registered Username” and the Name field should be displayed again. Either that or log out of WP and refresh the form. Just how it works man.
