user poll request form is not displaying

    The user poll request form is not displaying on Android tablet or phone shows fine on IOS devices. Running PM Version 8.4.6 and WP 7.1 on ForumVerse theme

    it isn’t a poll that is not displaying it is form for user poll submissions

    Here are screen shots first if of the user poll request form on android and the second on iOS. On iOS the request form is shown on Android it’s just white rectangle
