For your section “Question” you have the same URLs for your both languages. i.e :
“http://pursopal.com/questions-categories/” hreflang=”fa”
“http://pursopal.com/questions-categories/” hreflang=”en
With Polylang is not possible to have 2 identicales slugs. How did you that?
Please provide a screen copy of your english menu (Appearance > Menus).

Tobbar has problem
the Account arean “login” section has problem in Persian language when u login in Persian and want to go your “Answer” section in profile then it goes to english page automatically.
You change the language in Persian and login with this account and see :
user: mike
Pass: test
then click on the سوالات or پاسخ ها then u see that it goes to english automatically.
