[upload plugin]Private page and e-mail option

I’ve been searching high and low for this:

Upload plugin:

– text-fields: Title field, customer field, standard date field, (some custom fields?)
– No file-size limit and can only upload file-ext that the admin chooses (so .pdf .jpg but NO .exe of .rar files)
– Uploads are being posted on a page that the admins sets to private, so the client can check all the files and there titles when needed.
– Uploads are also send as attachment(s) to an dedicated e-mail_adres (or more e-mailadresses)
– several items for upload (max 5 items or so)

Can anybody help me with this?
