Upload image error 404

Hi there,

I have recently installed your plugin. However i have an issue whereby in the member profile page, i tried to upload my photo. however upon uploading it shows “error:not found”.

So when i inspect its element in console, i found this message:
Failed to load resource: the server responded with a status of 404 (Not Found)

What could be the issue?
