It is possible that your “uploads” directory is restricted. You may temporarily set the permission to 777 and retry.

It works with images but not with PDF. I tried also on a server of another host, but encountered the same problem. To clarify: The upload itself works, but to add a PDF to a booklet fails.
I can give you administrator access to a test installation if you are interested in.

I tried with a new WordPress installation and version 1.1.6, but it does still not work for me.

If there was an error during PDF upload, there should be a popup that displays something like “Command exited with error code: <error_code>”. What is the error code?
