Undefined variable

Hi,
nice plugin but in debug setup it says:
Notice: Undefined variable: form in /wp-content/plugins/sugar-events-calendar-ninja-forms-add-on/sugar-calendar-ninja-forms.php on line 51.

Can you give me help to fix that?
