I spot other problems with your form as well. When I select a practitioner I can no longer change my name or email. There’s also another unrelated error in the console. There’s nothing wrong with the form. I dug a bit deeper and was able to figure out the 2 problems.

Problem 1: The “unsubmitted value” error doesn’t appear anymore. I assume you have modified the practitioners list recently. Next time you edit the form, make sure to clear all your site caches.

Problem 2: when a practitioner is picked, an invisible box around the services dropdown stretches up and covers those fields, blocking clicks. I checked, and adding this style to your CSS should fix the issue: .wpcf7 form .wpcf7-form-control-wrap { display: inline-block; width: 100%; }. It confines each field’s wrapper to its own row, so the overlay can’t happen. No visual change otherwise.
