It looks like you just upgraded to 2.5. I believe the function that is undefined is supposed to be in wp-includes/media.php
I have checked media.php in my installation and found the function you are missing, wp_get_attachment_image() in media.php, so I’m not clear why you don’t have it. You can try re-loading media.php on the chance that it may be corrupted, but if it were I think you’d be seeing other issues also.
I’m not sure why it would be doing this…
I redownloaded 2.5 and deleted and re-added the wp-admin and wp-includes on the server and it’s still not working so I’m not sure what to do now…
I have been looking in the codex for answers, it is recommended that you re-load wp-settings.php. You’ll find it in your root directory for your blog. be sure that you save a copy of your original when you do something like this.
You’ll want to take it out of maintenance mode if you would like others from this forum to join in.

OH YES! Sorry! I’m pretty sure what fixed the problem was reloading the wp-settings.php file. Thanks for the help mechx1! I had never replaced the old file when I updated because the update only told me to replace my wp-admin and wp-includes folders. The wp-settings.php file was in the root directory and so I didn’t replace it with the new file included in the upgrade files for 2.5 – however I have done so now and it is resolved!

mechx1, you are awesome!!! I’ve been having a really hard time with the uploader. I did everything the mod’s sticky post said to do and still no luck. I even tried to disable the flash uploader but it still sucked. Now everything is working great with your tip. I hadn’t replaced the settings php because it was never mentioned in the upgrade outline.
