Unable to add Entry on created Application

Thanks for the good plugin. its great, i surely love it. i managed to install db-toolkit.0.3.2.026. Activated it and created the application and connected the interface to an existing database table. it allows to edit existing data. but when i try to add new data, it does not do anything, not even a form pops up. i looked at the browser console to see if there is any error : i found the following error. Uncaught SyntaxError: Unexpected token ILLEGAL /portal/wp-content/plugins/db-toolkit/data_form/javascript.php?ver=3.5:197

and

when i click add Entry i get this error :
Uncaught ReferenceError: df_buildQuickCaptureForm is not defined

Am new to wordpress, i don’t know how to proceed. please help me.
