Two select fields for date: year and month.

    The input type=date doesn’t work on Firefox or IE.

    Hey marianny, the free version doesn’t have Ajax… :/

    And unfortunately it is not possible to display dates like you mention, however the pro version uses a jquery date picker so users can pick any dates they want.
