Two Opt-In Checkboxes for one form???

Hi, there. Our site can be found at https://tufmed.com/. I am creating a form where the site owner asked that there be two opt-in options. If both are selected, the user should be added to two, separate lists within their Mailchimp account. Please instruct me on how I can achieve this functionality. As you can see, there are two opt-in checkboxes at the end of the form. However, they currently do not work as intended. Please advise.
