Tutor LMS Quiz Bug – 10 Questions Showing as 11 in Quiz Result

Hello Tutor LMS Support Team,
I am experiencing an issue with the Tutor LMS Quiz system.
I have created a quiz containing exactly 10 questions. The Tutor LMS Quiz Builder also confirms that there are only 10 questions in the question pool:
- Total questions in pool: 10
- Available to answer: 10
- Maximum questions per quiz: 10
- Question order: Random
- Time limit: 15 minutes
However, after a student submits the quiz, the Quiz Summary/Review page incorrectly displays 11 questions.
There is also an incorrect calculation in the result summary. For example, the result shows:
- 2 correct
- 9 incorrect
- 10 total
This is mathematically inconsistent because 2 correct + 9 incorrect = 11 answers.
Additionally, the Quiz Summary sidebar displays questions numbered 1 through 11, even though the quiz contains only 10 questions.
I have attached screenshots showing:
- The Quiz Summary where Question #11 appears.
- The Quiz Builder/Quiz Settings confirming that there are exactly 10 questions.
Could you please investigate this issue and let me know why Tutor LMS is generating/displaying an additional question in the quiz attempt result?
Please also let me know if this is a known bug and whether there is a recommended fix, update, or database cleanup required.
