Yes, this is a known issue. When you use a shortcode in a widget area vs. our widget this happens. However in the newest version of WordPress 4.9.1 this shouldn’t happen.
What version of WordPress are you on and can you try using our widget instead of a shortcode? https://yikesplugins.com/support/knowledge-base/adding-forms-to-sidebars-and-other-widgetized-areas/

Well, I’m using 4.9.1, I’ve just double-checked.
Unfortunately I can’t use your widget instead as my theme only allows 1 widget per area in the footer and I display more info (social media) below the subscription form (not visible on my screenshot because of the space that was added), so I’m using an HTML widget to display all of it.
Too bad, I guess I’ll just have to change the placement for this widget then and place it in a sidebar for instance.

I just tried testing the short code in a text widget (using Twenty Seventeen) and that worked great. When I try it in a HTML widget it shows just the shortcode itself.
I don’t know if you could try your code in a text widget? Maybe that will work.
Sorry, my mistake: I was using a text widget, not an HTML one. But I’ve tried with the HTML and it worked 🙂
