Theme Just Disappeared

I installed a custom theme that has been working great for several weeks. Last night my client was “clicking around” the dashboard, and the theme disappeared. She swears she didn’t do or change anything (ahem). The theme files are still on the server where they always were, but it doesn’t show up in the dashboard. Has anyone seen this before? What could have happened?
