Can you please clarify which form plugin you use along with E2Pdf?

If you are using Formidable Forms PRO, it must be possible to accomplish the task by using Conditional Statements (Premium Feature of Formidable Forms) inside “Success Message” or “Mail Body”: https://formidableforms.com/knowledgebase/conditionals/
