TDO MiniForms – can’t access the form

    I installed TDO MiniForms, but when I click on the link in the admin panel to view the form, it goes to “example.com/?page_id=5”, the site’s home page. The same thing happens when I go to the blog and click on the “submit message” link. What am I doing wrong?
