Q1> Can’t answer. Too many variables AND we’re not supposed to pitch recommendations for hosting (which includes CDN). They all work equally well in my experience.
Q2> Caching and server optimization. I don’t use CDN at all, just APC and a lot of time fiddling with my httpd.conf settings.

Q3 is 100% dependent on which CDN you pick.
You can probably use W3 Total Cache with any of them.

I installed W3 Total Cache but it says “plugin cannot be activated network-wide.” I’m looking for network-wide plugin, which can be controlled by Network Admin only..
In my experience so far Quick Cache plays nicely on multi-site/multi-network installs.
If Quick Cache is installed under a Multisite/Networking installation, it will be enabled for ALL blogs the same way. The centralized config options for Quick Cache, can only be modified by a Super Administrator operating on the main site. Quick Cache has internal processing routines that prevent configuration changes, including menu displays; for anyone other than a Super Administrator operating on the main site.
