— What version of the List Subscribe plugin is being used?
— What version of WordPress is being used?
— What is the name of the theme being used?
— Does the issue persist if you temporarily switch to a different theme?
— What are the active plugins running on the site?
— If possible, could you try to deactivate all plugins except for the List Subscribe plugin to see if the issue persists when only the List Subscribe plugin is active?

Thanks for going through these troubleshooting steps so far and sharing all of this information with us. I’d like to try working towards replicating this issue on our end.
In order to make sure we’re testing thoroughly, would you be able to send us a screenshot of the Plugin settings for the List subscribe plugin as they appear on your admin console? You can find these settings by going to the WordPress Admin Panel > Plugins > MailChimp > Settings.

Thank you for sending us all of that information! I went ahead and passed these details along internally to allow a dev to take a closer look. I’m hoping that this may simply be a theme or plugin conflict, but I was unable to locate all of the plugins listed on my end to test. Do you mind replying back here with a screenshot of your installed plugins? That may help me out a bit. Also, could you try temporarily reverting your theme to a different WordPress theme to see if that causes any changes in plugin behavior? It seems to more likely be a theme conflict, as you mentioned you’ve already tried temporarily deactivating all of your plugins but this one, and that did not cause any change.
