You are using the free version of the plugin, and the free version of the plugin does not include a submit button by itself. If you want to include a submit button in the free version of the plugin, you must insert a button field in the form with the following piece of code as its onclick event: jQuery(this.form).submit();
But note the free version of the plugin does not include the server-side modules to process the submitted information, like to send the notification emails, store the information in the website’s database, integrate the form with a payment gateway, etc.
