Submit Button and Text Issues

Hi,
Please help me.. My submit button somehow went next to the upload file button.. Previously it was under the upload file button.

Plus if you look at the checkbox, “Your Message” text should be shifted down not mixing with the checkbox refund option.
Please help me solve this.
