One of the common reasons for such issue is a “mixed” mode of the gateway – for example, setting gateway to “live” mode having only “test” keys configured (in “Formiantor -> Settings -> Payments” page) or the other way around.
The second common reason, actually happening more often, is caching. Is there any cache active on site/server? If yes, try clearing it up and temporarily disabling to see if the issue goes away.
If this helps then try first enabling “Prevent page caching on form pages” option in your form’s “Behaviour” settings and if that still doesn’t help, try excluding the page with the form from cache entirely (there might be different ways of doing that, depending on what caching plugin are you using).
