Split profile form Plugin

    Hi,
    i just installed and activated the split profile plugin, i duplicate the default profile form but i’m not able to see it on profile page.

    What am i missing?

    but sorting is strange. Seems like decreasing ID form number, but the first i created it’s still the first, maybe becouse i declared it on Profile wordpress page…
