I guess you want to install an anti spam plugin. Akismet is already in your plugins directory if you use 2.3.x, otherwise there are numerous different ones. I use Protect Webform (with a captcha image for confirmation) which works perfectly.
You need to enable the moderation function. I have great respect for Akismet. But there will be a lot of spam comments that will evade Akismet. Or you want to perhaps add other anti-spam plug-ins.
It must be some kind of exploit or something though. I clearly have it checked saying “must register to comment”. These comments have never registered.
I just put on akismet last night. So I hope it helps. But kinda worries me of exploitation of wordpress.

Negative. And it doesn’t really matter in this particular subject. Spammers download WP package and analyze software vulnerability. If you use moderation, your viewers will never see spam comments as long as you block them all manually. Akistmet and Bad Behavior will only stop comments from rookie spammers.
Moderation is the only perfect solution. That’s because we are smarter than computers. Computers are faster but not smarter.
These comments have never registered.
That’s because they’re not comments. They are trackbacks or pingbacks. They look like comments and act like comments, but come in through a separate door.
Akismet and Bad Behavior stop virtually all of them. I’ve never needed moderation with those two running.
