If not, a link to your site and a description of what’s happening vs. what should be happening would be helpful so we can determine what the problem is.

This is what I did –
I went through and did the questionaire.
Even though it made the pages the pages could not be displayed.
So I C&P’d the data on to new pages and it was then fine.
