shortcode is not working

I tried using the shortcode but it has absolutely no output. My page outputs fine with no discernible errors, but the shortcode area yields no form. I’ve noticed two other similar posts to mine on this forum but with no real followup. Could anyone please chime in with how they solved this problem if a solution exists? Thank you.
