Shortcode Form Not Processing

Hi There,

I am having trouble with a page that I have embedded a form into using a shortcode. The form displays fine but when I go to input content and press submit, the form simply refreshes and does not submit the content or go to the page it is suppose to after submitting the information. The strange part is the form works perfectly fine as a widget in the side bar.
