1. Make sure the plugin is installed and activated
2. Edit the page and view it in text mode and make sure there are no code or pre tags around the shortcode, or any other weird formatting characters. Delete and type in manually if needed instead of copying and pasting from somewhere else.
3. Alternately, install the free shortcodes plugin to generate the shortcodes properly for you:
