Hi, i think that i have the same problem.
We have a problem with the shipping setting: we update the woocommerce to the 2.6.6 version and we re-create the shipping method with the new shipping zone but we can se only the first ship of the list, if we change the order the first element of the list is visible in the front end. To understand the problem we have installed an old version of woocommerce and the shipping method was ok. Than we delete all the shipping setting, update to last woocommerce and create again with the shipping zone but was the same, in the front we have only the first shipping of the list.
Do you know how I can resolve?

You have one shipping zone and 3 options like flat rate?
Ok. You can check:
