Setup Guide?

    This plugin sounds like it’s exactly what I need, however the doccumentation seems to be somewhat lacking.

    I’ve managed to create a template that I like, but I can’t for the life of me figure out how to connect it to a form. Is there a setup guide that’s a little more detailed than “Create a new Template, activate and use one of the shortcodes available to add PDF to needed page/form and you’re done”?

    I could probably even figure it out on my own, but it doesn’t even say which form plugins are compatible with it. Does anyone have any suggestions? Preferrably one that’s free, since I’m on a tight budget.
