I am on WP 3.5. using the Responsive Theme with a heavily modified Child Theme.
I actually got it to work on a blank template page, but I’m having problems displaying it in a SImplemodal modal because it ends up also loading a full page with headers and footers I don’t need; I just need, to load the form.
Will keep you posted on developments.

Also, as a follow-up, the blank template I’m using is literally blank. I’m not calling any headers or footers.
I tried Responsive By ThemeID Version 1.8.7 whiteout problems.
Have you set
Set Login Type: “Would you like to use the default WordPress page and have the ability to change the logo and colors?”
because the behavior seems to be that of the other option…
