Searching a form plugin or report plugin

Hi everyone,

I am searching a plugin for reporting.
I would like that a user that is registered (name, password, description) can give informations with a form and it is automatically saved in database.
The user can put numbers in a specific field like if it was a wear saler:

User 1
Field: tshirt, Number: 1000
Field: sweats, Number: 1200

User 2
Field: tshirt, Number: 500
Field: sweats, Number: 750

Those results are saved and it can be seen in a admin panel
for the results of all the wear salers:

Field: tshirt, Number: 1500
Field: sweats, Number: 1950

The admin panel resumes all the results and it can be exported with excel files like in xls.

I don’t know if this kind of plugin exists, i have not find this plugin,
but if you know where is it, it will be very helpful for me.
