Your going to have to post some code snippets of the header your using and the corresponding css.
We cannot help you until you do this. I can only tell you what code your going to use to display the form, but I’m guessing you already know that code. So the next step is displaying it in the right spot.

As per forum rules, please use the pastebin to post long pieces of code — they don’t want to clutter up the forums.
You have to use the code button when posting before, then when the code is over, hit the code button again.
