Return form – complex contact form

My client would like to make a return form, basically I am looking for a contact form which has a multi selection, when selecting one of the two options, depending which you select will determine the input after that.
For example: http://www.q-imaging.com.au/return-form here when you select faulty goods the forms below change to suit that and when you select return product again it changes.

I’m not sure if this is possible, but any help to achieve this would be greatly appreciated
