Reset Password Page Blank

Hi,
In the settings, I set the “Password Reset Page” to the page “NewPasswordNeeded” and am currently getting a blank page. The page is: http://dedicated.floridalibrarywebinars.org/newpasswordneeded/
Is there a shortcode or something else I am missing?
