Quick Contact plugin error problem

hello. I have a organic theme & I have added the quick contact plugin. I managed to get it added to my control panel & it has come up on my site. however, after I test it, I continue to get an “error on page” message on the bottom of my browser & nothing comes to my email AND the page seems to lock up with the “SUBMITTING MESSAGE, PLEASE WAIT” message.
