Can you perhaps provide a link to your site so I can take a look and possibly see why that happened?
Normally it’s because another plugin causes conflicts when the uploads or the edits run.

I can see you’re also using PageSpeed on the server, as well as RevSlider.
The only image I see broken on the main page is http://www.modernhappyhome.com/wp-content/uploads/2015/02/Feng-Shui-Agency.jpg (which doesn’t exist). Is there a page with the broken images?
Looking at your DreamObjects setup on your Panel at DreamHost [deleted] Whoops wrong account. I see you’ve got the objects loaded up to the cloud and they seem to work though it looks like they’re set private?
