Problem with media library / image upload

    Hi,

    lately I’m having troubles with uploading pictures to the media gallery.

    When I upload one over e.g. Image gallery I get an error:
    http://www.stylebug.cc/test/1.jpg

    The img is not stored in uploads folder

    When I switch to Media library, it looks like this:
    http://www.stylebug.cc/test/2.jpg

    When I upload a pic on media library it is stored in uploads folder but media overview looks the same as on screenshot above.

    I’ve already deactivated all plugins and switched to twentytwelve template – the error still appears.

    The images are not big, about 1500x1000px and under 1MB size

    I’m not sure if this problem is new since update to WP 4.0 (I didn’t upload anything right after update).

    Hope anyone can help.
