Post Notification not working

Help! I did not create this site but they asked me to make some changes. I transferred the “Sign up to receive posts via your inbox” from the left sidebar to the right and now it’s not working. I copied all the code. I’m not sure what it’s not working. Three things are happening:

1.) Several subscribers who were receiving all the posts say they are no longer receiving them….however, I am subscribed to all categories and have received them all so have no idea where to start with this.

2.) When you add your email address and then get the email confirmation and click the link, the link goes to the homepage. It used to take you to a page to choose your categories and confirm but have no idea where it is. You can’t do anything.

3.) The unsubscribe link is also going to the homepage instead of an unsubscribe page.
