Do me a favor and look inside your WordPress Dashboard > Comments. Do you have “By [name]” as the name field for that comment?
Have you made any modification to WPBook?
What’s the link you your app/wall?
I haven’t made any modifications to WPBook. Under the comments field, it just has the name. There’s no “By” for any of the comments.

I’m not seeing the issue in your Facebook app which tells me it’s not a WPBook problem but a problem with your parent theme. I would check your theme (outside of Facebook) for an extra “by.” As I don’t see this as a WPBook problem I’m not sure I can be much more help other than to advise you check your parent theme.
You may want to change your parent theme to the default (twenty-ten) and see if the problem goes away. If so you know it’s a problem with the theme.
