Are you sure that minify is enabled on the general settings tab and CSS and JS is enabled on the minify settings tab?
Ah @frederick, thanks – in preview mode, they both enabled, but live on the site, CSS Minify was enabled and JS Minify was NOT enabled.
So I renabled JS Minify, cleared the cache, and the setting for hosting the JS/CSS Minify files with CDN is still greyed out. Here is the screenshot:

And I’m still getting the vague error message about the missing files.
Thanks,

This option is only available when rewrite minify URLs is enabled.
I came across this excellent guide yesterday. It might help explain a lot to those new to W3 Total Cache.
http://zemalf.com/1443/w3-total-cache/ – I recommend checking out the e-book zemalf has on scribd. The link is on this page.
Additionally this post helped to explain opcode caching and setup APC. http://www.virtualmin.com/node/15258#comment-69432
Thanks for the direction, @frederick – after following your suggested procedure, I tried deleting the main stylesheet: wp-content/themes/bp-fun-child/style.css from the minify settings in order to get the site to display properly.
@frederick, are you sure this isn’t a bug in W3TC? I have a WP site that I just moved from an server running apache to a server running litespeed and I ran into the exact same problem. I was my main style.css that was causing the issue. The wierd thing is that I have 2 other wordpress installations that I moved to another litespeed server that had the exact same problem, and all were running W3TC with different themes.
Do you know what could be causing this?
my blog is thepowerofbeing.net and I am trying to activate my scribd plugin after updating to 3.2.1 and I get the following error message:
Fatal error: Cannot redeclare ipaper_head() (previously declared in /home/dmolini/public_html/wp-content/plugins/wp-ipaper/scibdipaper.php:103) in /home/dmolini/public_html/wp-content/plugins/simpler-ipaper/scribd-ipaper.php on line 71
Please tell me what to do
@dmonlini, I’m not convinced that error is related to W3TC.
