Plugin to showcase products and ordering system

Hello,

Can anyone point me to a plugin that can do the following:

1. Create a product showcase/catalogue that shows information on each product with images, text etc etc.
2. And more importantly, the showcase includes an order form that the user can fill out for a specific product or range of products and this form is sent to the administrators to process further as they wish.
3. Incorporate flash in the product pages to showcase images, videos etc in place of the regular html image based product shots.

I know that most ecomm plugins do the first part but I am not aware of any plugins that can incorporate a simple order form in place of the online and offline payment systems.
