Plugin to create New link and page

I want to develop a plugin which can create a new link and a page in any theme. But i do want to create any template page or wordpress page like we add from dashboard->pages.

This page will be dynamic. I want to keep code to generate the page in plugin folder. So that there should be no need to add any thing, just install the plugin and it can work.

How I can achinve this any idea.
