Plugin that inputs a post title into a contact form

I’m creating a volunteer database for my church. Each job is listed in a custom post. I’d like to create a link that would take someone to a form for sign-up. I don’t want to create a specific form for each position. Is there a plugin that would auto fill the form with information from the post? i.e. the title of the post (which is also the title of the volunteer position) into the form.

I want them to be able to click on a “sign up here” button. It would take them to a form that is designated to be emailed to someone. They would input their personal information. But the title of the post would be auto-filled for them.
