Plugin suggestion for contact form CRM

Hi!
We will have a B2B sales contact form on our landing page which will store each filled in entry into a database table. Does someone know of a CRM that could read from the database table, and display each entry as a sales ticket, to manage leads this way? Some wanted functionality:

* Move between status categories, ie. “New” to “In Progress” to “Closed”.
* Assign employee to handle new open tickets
* Adding comments, create sales pipelines for each ticket etc.

Ideally a simple solution where each entry/ticket can be expanded to reveal contact information as filled in with the form entry, then possibility to follow up each individual ticket as separate cases, move them through categories etc.
