[Plugin: SimpleMap Store Locator] General Options is blank

I’m using the latest SimpleMap plugin. I encountered this first at version 3.3.2. I’m running a multisite installation and the weird thing here is it just blanks out on one site. I have it activated on two other sites and there are no problems.

Aside from General Options, the Import/Export CSV page is blank as well.
