Plugin only intermittently submitting to Mailchimp

I’ve installed this nice plugin to work with Contact Form 7. It seems to only submit the subscriber info to Mailchimp intermittently, less than 50% of the time. Do you have any suggestions or thoughts on what might be the issue?
