[Plugin: Jazzy Forms] Basic how-to user guide?

Hi,

I am all new to Jazzy Forms as well as being a total newby to website stuff in general. I want to put a quote form on my site that can allow people to select from a series of drop down menus kind of like this http://eplot.com.au/flyer-printing.htm?gclid=CJyRp9iKtrACFSRKpgoddl7X8w

I’m not sure if Jazzy Forms is appropriate & I can’t work out where to start! Is there a step by step guide for dummies?!
