PlugIn Hunt – HELP!

Greetings.

I am hunting for a plug that will allow me to do the following:

1. A site visitor requests a white paper
2. The site user must fill out a request form to get the white paper
3. The user gets an autoresponder (either an email or a new page)
4. User gets white paper and I get a new prospect

Admin wish list:
1. It would be nice if I could set up multiple instances of this. For example, if I have three white papers, I would need three versions of the autoresponder with custom links in each.

I have been hunting for this but can’t locate one here. But that doesn’t mean they don’t exist.
