[Plugin: File Gallery] How do I set up "Template: Simple (with javascript)"

I can’t seem to locate the instructions to create the “Template: Simple (with javascript)” example found here: http://skyphe.org/code/wordpress/file-gallery/file-gallery-examples/
I’ve tried all the different gallery settings but could not figure out how to implement this.
